Primary Care Referrals and Scheduling
One of the first steps in registering a client is ensuring they have a PCP or Primary Care Physician. This is essential to their overall quality of care and proper coordination of services. If a client does not have a PCP, a Care Manager will refer the client to one within our network. The CM will also ensure the PCP appointments are scheduled and transportation needs are arranged if necessary. We stress the importance of our clients communicating their PCP appointments with their Care Managers, which allow them to conveniently manage their services and referrals.
Care Managers are often first called when there is a medical crisis. Care Managers are clients’ front-line advocates who know the medical system, know the client, and can help provide the best care. Before a crisis occurs, a Care Manager knows a client’s full medical history, records all the prescription medications, and has attended doctor visits with a client in the past. They know how to ask questions and bring up concerns with a doctor and can summarize and take notes of the encounter to be discussed afterward with all interested parties.
