Sapient Medical is a company with a dynamic working environment that is looking for a Licensed Practical Nurse (LPN) to join their community based behavioral health clinical team. Working with new and routine patients in an office or community setting allows providers to efficiently treat their patients. The LPN duties include assessing patients, conducting evaluations, scheduling services, and making community referrals according to the patients’ needs. Other duties include administering medications to aid the patients’ health and using medical equipment to monitor vital signs. The LPN is also responsible for working with providers outside of the facility to help treat the patients’ total health.
Licensed Practical Nurse Job Requirements and Responsibilities:
- Schedule and coordinate patient services, treatments, and training.
- Observe and record patient behavior.
- Collect patient health history.
- Documents patient care services by charting patient and department records.
- Perform physical assessments, evaluations, and diagnostic tests.
- Counsel and educate patients and their families on treatment plans.
- Administer medication, and train patients and their families on medication administration.
- Read and interpret patient information, making treatment decisions if appropriate.
- Collaborates with physicians and multidisciplinary team members.
- Provides physical and psychological support to patients, friends, and families.
- Identifies patient care requirements by establishing personal rapport with patients and their families.
- Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
- Maintains nursing supplies inventory by checking stock to determine inventory level.
- Call patients, their families, PMP, specialists, and other providers to schedule, coordinate, update, and manage appropriate care.
- Visit patients at their homes, in their community, at schools, and other treatment facilities to provide nursing services according to a schedule and treatment plan.

Physical Requirements and Work Environment:
- Environment: Work is performed in an office, home, or community setting with little patient contact. Due to the nature of the work, the LPN may be exposed to bodily odors on an occasional basis.
- Physical: Primary functions require little physical ability and mobility to work with a patient; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 25 lbs.)
Education, Experience, and Licensing Requirements:
- Appropriate level of entry degree to practice as a license LPN in the State of Georgia
- Maintain CPR and First Aid certification, and annual TB screening.
- Active state of Georgia Practical Nurse or Registered Nurse Licensure
- Clean background check and clean drug screen
- At least (6) six months skilled nursing experience
License/Certification:
- LVN/LPN (Preferred)
- BLS Certification (Preferred)
