Facility Nurse’s (RN/LPN) Job Requirements and Responsibilities:
- Intake/enrollment
- X visits per day
- Treating patients including children and adults
- Supervise PCA and HHA and evaluate staff providing care through on-site supervisory skills.
- Recording and reporting aids performance.
- Assist aides with adapting work situations.
- Interpret policies and procedures to patients, family, practitioners, and others.
- Communicate with the Clinical Director on a routine basis.
- Advising patients on health maintenance and disease prevention or providing case management.
- Assisting in quality assurance measures by maintaining documents for Article 28 Clinics
Additional RN/LPN Responsibilities for Facility Nurse
- Keeping employee charts up to date
- Maintaining inventory for medical supplies
- Providing education on employee wellness
- Drawing bloods
- Administering vaccines
- Assuring all medical/nursing needs of residents are addressed.
Desired experience for registered nurse, RN/LPN includes:
- Providing routine and emergency nursing care to consumers.
- Training and monitoring Direct Care staff on assigned aspect of consumer health care (includes medication and administration)
- Maintaining nurse’s notes and other appropriate documentation of activities as appropriate.
- Preparing assigned consumer Annual Quarterly and Special case review nursing summaries.
- Participating in consumer medical appointments.
- Ordering/reordering medication from pharmacy as required.

Physical Requirements and Work Environment:
- Environment: Work is performed in an office, home, or community setting with little patient contact. Due to the nature of the work, the RN may be exposed to bodily odors on an occasional basis.
- Physical: Primary functions require little physical ability and mobility to work with a patient; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 25 lbs.)
Education, Experience, and Licensing Requirements:
- Appropriate level of entry degree to practice as a license
LPN/RN in the State of Georgia
- Maintain CPR and First Aid certification, and annual TB screening.
- Active state of Georgia Registered Nurse Licensure
- Clean background check and clean drug screen
- At least (6) months experience
License/Certification:
- LPN/RN (Preferred)
- BSN/APRN (Preferred)
- BLS Certification (Preferred)
- (1) one year experience (Preferred)
