Sapient Medical is a company with a dynamic working environment that is looking for a Care Manager to join their community health team of clinical professionals. The Care Manager facilitates community health services and clinical events in the office and within the community. The Care Manager is primarily responsible for scheduling and coordinating patient services. The core job is to ensure patients are available, scheduled, and present to receive their clinical/medical services from the providers in the clinic or in the community. A key role in this job is organizing the community events that allow the residents to receive nursing services, registering patients, scheduling patient appointments, creating nurse schedules, assigning nurses to patients and making patient referrals.
Community Care Manager Job Requirements and Responsibilities:
- Register and conduct intake screenings on new patients.
- Schedule and coordinate services for individual patients and groups.
- Set patients on nurses calendars for In-Clinic, Out-Patient, and Telehealth treatment.
- Make telephone and telehealth calls to patients to coordinate, schedule, provide services.
- Conduct telehealth calls to educate patients and and follow up on their treatment.
- Provide community resource management services by making appropriate referrals.
- Chart or document all services and treatment within the designated EMR
- Review treatment plans with patients and educate patients and their families on best practices.
- Call patients, their families, PMPs, and other providers to help manage patients’ care.
- Identify community organizations to refer and coordinate events for community health.

Physical Requirements and Work Environment:
- Environment: Work is performed in an office, home, or community setting with moderate patient contact. Due to the nature of the work, the clinicians may be required to travel to community facilities, homes, satellite offices.
- Physical: Primary functions require little physical ability and mobility to work with a client; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 25 lbs.)
Education, Experience, and Licensing Requirements:
- Appropriate level of degree or certificate in the State of Georgia
- A State of Georgia driver’s license
- Maintain CPR and First Aid certification, and annual TB screening..
- Clean background check and clean drug screen
- At least (1) years of community health experience
