Dental Care Referral and Placement Services

We know regular oral health checkups can reveal clues to your overall health and spot potential problems. Oral health has a great impact on your quality of life and may affect your livelihood. This is why it is one of our key areas of community health services.

 SM helps clients identify and receive affordable routine dental services. Some options consist of them signing up for a qualified program, while others may include participating in mobile care dental services, accepting pro bono treatment from local dentists, or attending free community programs provided by the state or non-profit organizations.  Our network provides services to those who can’t afford dental care, including youth, older adults, and homebound seniors.

The Care Manager’s objective is to ensure clients are aware, have access to, and scheduled for affordable dental services. Care Managers know which option is best for the client because they have registered them for other services, understand their strengths and needs, and know what resources are available at the required time.  The Care Manager has the best overall view of the clients’ needs and their ability to participate in any treatment or services.
