Community Health Programs

Sapient Medical also provides health education, disease prevention programs, mobile health clinics, community outpatient, and enrollment services to local and state residents. These services are the basis of our community health programs so we are inspired to inform as much as we are to perform.

We know there is an information gap between available and affordable healthcare, and we understand there’s an outcome gap between referring and encouraging healthcare services. Our community health programs help ensure these gaps are closed because referring without encouraging may often be discouraging.

Community Health Services Requirements

We accept all forms of insurance and strongly believe our clients have the right to quality care that meets their needs regardless of ability to pay. Our network of health care organizations is friendly, professional, and efficient.

Our Care Managers and staff work together to provide consumers with the best and most comprehensive services available. Our goal is to facilitate excellent quality of care for EVERYONE regardless of insurance status or ability to pay.

We do not charge clients for our services, but we do verify all clients for insurance purposes.  This prevents us from referring a client to free services that may be covered by their insurance. It also allows limited space opportunities for treatment to remain available for those who do not have insurance.

Those who have insurance have more options than those who do not. It is important to refer clients to community resources that align with their insurance, which allows their treatment to be easily coordinated, and not denied or duplicated.

On behalf of our network, we accept most types of insurance including Medicare, Medicare Advantage Plans, Commercial insurance plans, Medicaid, Ameri Group, Care Source, Peach State, Tri-Care, and others.

If a community resident or consumer is associated with a client or partner of the community health program, we post a sliding scale fee schedule used by most of our providers. Sliding fee discounts are determined by an application process that can even be completed on your first visit or participation in one of the programs.  This fee is not imposed by Sapient Medical, but by regulating authorities of the provider required for indigent and uninsured clients participating in their network. Most of the time these fees may still be waived.
