Sapient Medical is a company with a dynamic working environment that is looking for a Care Manager II to join their community health team of clinical professionals. The Care Manager II facilitates community health services and clinical events in the office and within the community. The Care Manager II is primarily responsible for scheduling and coordinating patient services.  The core job is to ensure patients are available, scheduled, and present to receive their nursing/medical services from the providers in the clinic or in the community.

The Care Manager II oversees the master patient list to ensure all nursing services are provided to each patient. The CM coordinates and schedules each patient for nursing services according to the standard requirements and the patients’ medical needs.  The CM creates the calendar and is responsible for contacting the patients and families to ensure they receive the treatment on the days and times most convenient for them.   They also contact the patients to ensure proper notice is given to them if there are any changes to the schedule or their treatment.

One of the most important roles of the CM is to ensure that each nurse is available and scheduled to treat the patients. The CM assigns the service calendar to the nurses on a weekly basis, sometimes daily for walk-ins or new patients. Additionally, they are responsible for monitoring the nurses time and location to ensure they are available and not over or under booked. If the CM is properly credentialed, they will schedule themselves on the calendar and pick up any overflow or backlog of patients.

Care Manager Job Requirements and Responsibilities:

  • Register and conduct intake screenings on new patients.
  • Schedule and coordinate nursing services for all registered patients.
  • Create the nurses’ weekly and monthly schedules and assign their patients.
  • Set patients on the nurses’ calendars for In-Clinic, Out-Patient, and Telehealth treatment.
  • Monitor nurses’ time and location to ensure they are not under or overbooked.
  • Provide nursing services for overflow and backlogged patients.
  • Make telephone and telehealth calls to patients to coordinate, schedule, provide services.
  • Conduct telehealth calls to educate patients and and follow up on their treatment.
  • Provide community resource management services by making appropriate referrals.
  • Chart or document all services and treatment within the designated EMR
  • Review treatment plans with patients and educate patients and their families on best practices.
  • Call patients, their families, PMPs, and other providers to help manage patients’ care.

Physical Requirements and Work Environment:

  • Environment: Work is performed in an office, home, or community setting with moderate patient contact. Due to the nature of the work, the clinicians may be required to travel to community facilities, homes, satellite offices.
  • Physical: Primary functions require little physical ability and mobility to work with a client; to stand or sit for prolonged periods of time; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull light to moderate amounts of weight (up to 25 lbs.)

Education, Experience, and Licensing Requirements:

  • Appropriate LPN / RN level of degree or certificate in the State of Georgia
  • A State of Georgia driver’s license
  • Maintain CPR and First Aid certification, and annual TB screening..
  • Clean background check and clean drug screen
  • At least (1) years of community health experience
